This form is to be used only by case workers, staff members of
non-profit social agencies and clergy seeking appointments for
clients who need our help to relieve hardships.
Requests must be submitted from the social agency
email address, not a personal email address. Please confirm that your client has a true need for basic items. It is our policy to allow each guest only two visits. The first is to select needed items from available inventory. If the guest is unable to remove those items at the end of that visit, a second visit will be scheduled so the guest can arrange for transportation and look for needed items not available during the first visit. The depot is staffed by volunteers on Tuesdays and Saturdays, but appointment requests are usually acted on within 48 hours. (Note: Some clients have been scheduled for appointments but failed to show up and failed to call in advance to reschedule. We call those "no-shows" and will not risk a second appointment for them. If an agency has had more than 2 no-shows in the previous 30 days, we will not schedule an appointment unless it is stated in the "Comment" box that the caseworker will accompany the client to the appointment. If we find that your agency has had multiple no-shows, you'll be asked to resubmit the request.)
